Prior to making an appointment, it is strongly advised that you call your insurance company to find out if you need prior authorization for treatment. If you do your insurance company will provide you an authorization number. Please take that you with at your first visit. Find out how many sessions your authorization is good for and find out if you have a copayment for your sessions, how much will be your responsibility.
Some services you may want to receive from me may not be covered under your insurance plan. Please be away of these services for they will become your responsibility for payment. Hypnotherapy sessions are only self-payment. Payment options need to be addressed prior to treatment.
